Eligible Medicare beneficiaries in Iowa and across the United States are set to receive a one-time $90 payment in October, with most direct deposits expected on or around Oct. 8.
The Medicare Improvement Fund Premium Rebate is available to an estimated 20.8 million people nationwide enrolled in Original Medicare Part B who meet the federal eligibility requirements.
People enrolled in Medicare Advantage are not eligible for this payment. The rebate also excludes Original Medicare beneficiaries who receive Medicaid assistance paying their Medicare premium or who pay an Income-Related Monthly Adjustment Amount, known as IRMAA.
The payment is intended to offset part of the October Medicare Part B premium. It is a one-time $90 rebate, not a permanent reduction in monthly Medicare premiums.
Most $90 Medicare payments are expected around Oct. 8
Most eligible beneficiaries will receive the $90 payment by direct deposit through the Social Security Administration on or around Oct. 8, 2026.
Beneficiaries who do not use direct deposit will receive a paper check from the U.S. Department of the Treasury later in October at the mailing address registered with Medicare.
The paper check will identify the payment as a Medicare Improvement Fund payment intended to offset the October premium.
Original Medicare Part B beneficiaries must meet additional eligibility rules
Enrollment in Medicare Part B alone does not guarantee the $90 payment.
The federal eligibility criteria apply to people enrolled in Original Medicare Part B who live in the United States and meet the program’s other requirements.
- They must be enrolled in Original Medicare Part B.
- They must live in the United States.
- They cannot be receiving Medicaid assistance with their Medicare premium.
- They cannot be paying an Income-Related Monthly Adjustment Amount, or IRMAA.
CMS has not published an Iowa-specific estimate for how many beneficiaries in the state will receive the rebate.
Medicare Advantage members will not receive the $90 rebate
People enrolled in Medicare Advantage are excluded from the Medicare Improvement Fund Premium Rebate.
This distinction is important because Medicare Advantage beneficiaries still generally have Medicare Part B, but their coverage is provided through a private Medicare-approved plan rather than Original Medicare.
The $90 payment applies specifically to eligible beneficiaries in Original Medicare Part B.
The rebate does not permanently lower the Part B premium
The standard Medicare Part B premium for 2026 is $202.90 per month. The $90 payment offsets part of one month’s premium but does not change the standard monthly premium for the rest of the year.
CMS describes the payment as a single Medicare Improvement Fund Premium Rebate for October 2026.
1-800-MEDICARE can verify rebate eligibility
Beneficiaries who are unsure whether they qualify can call 1-800-MEDICARE (1-800-633-4227) to check eligibility for the $90 rebate.
People who want to check the status of a payment can contact the Social Security Administration at 1-800-772-1213 beginning Oct. 15.
CMS says eligible beneficiaries will receive the rebate through direct deposit or a Treasury check using the payment and mailing information already associated with their Medicare and Social Security records.

Most direct deposits are expected on or around Oct. 8, while paper checks will arrive later in October. Social Security will begin answering payment-status questions on Oct. 15.
